Please submit this form if you wish to close your Affiliate account. When your request is validated and approved, you forfeit any commissions, bonuses, incentives, refunds, or any other transaction due. You also cancel your relationship to any other Affiliate in your Stack. Affiliates from your Stack will be reassigned to another Sponsor. This is final and cannot be rescinded or undone.
